Commonwealth Home Support Program (CHSP)
The Commonwealth Home Support Program (CHSP) is the starting point for those aged 65 years or older (or 50 years or older for Aboriginal or Torres Strait Islander people), to receive in-home and community support services that enable you to continue living independently and safely in your own home.
What we offer
West Wimmera Health Service is funded to provide the following CHSP funded services to people living in our catchment:

Allied Health and Therapy Services - healthcare services provided by professionals such as a physiotherapist, occupational therapist, speech pathologist, podiatrist, dietitian, exercise physiologist, and social worker. This also includes hydrotherapy.

Flexible Respite - support services designed to provide temporary relief and support for caregivers.

Home Modifications - adaptations and alterations made to the home to improve accessibility and safety.

Nursing (Community Nursing) - nursing services either provided to you in the comfort of your own home or delivered within our West Wimmera Health Service facilities.

Social Support - Group - group activities and socialisation opportunities held at your local centre and off-site destinations. Transport is provided as part of the program plus meals are supplied for all centre-based activities.

Specialised Support Services (Continence) - specialised assistance and resources to manage continence issues and promote bladder and bowel health.

West Wimmera Health Service is also funded to provide the following additional CHSP support services to people living in the Hindmarsh and Yarriambiack Shire areas (Nhill, Jeparit, Rainbow, Dimboola, Minyip, Murtoa, Rupanyup and surrounding areas):

Domestic Assistance - assistance with household tasks such as cleaning, laundry and shopping.

Home Maintenance - basic home maintenance and gardening to maintain a safe home environment.

Meals - supply and delivery of meals to your home or at an organised centre base or assistance with meal costs through vouchers that are redeemable at participating food outlets in your town.

Personal Care - assistance with daily living and self-care tasks like eating, bathing, dressing, and medication.

Social Support - Individual - one-on-one support and companionship provided in the home or community for example spending time with you in your home or out and about for leisure or attending appointments and paying bills.

Transport - assistance with transportation to appointments, social activities, and usual outings.
​Please contact your local Shire Council if you are seeking additional support services and you don’t live in any of the towns or regions listed above.
How to access
To access CHSP services you need to contact My Aged Care on 1800 200 422 and request an assessment. Our staff are here to support you with this step, so please call us and we can start the process together.
Cost
The cost of CHSP services is partially subsidised by the Australian Government however the program also requires you to contribute to the cost of your care if you can afford to.
We endeavour to keep our client contribution fees affordable and use a simple method of concession and non-concession status. Proof of concession is provided by showing us your pensioner card. If you are concerned about the cost of services, please let us know and we can help you apply for financial consideration.
