Hospital in the Home

West Wimmera Health Service provides Hospital in the Home (HITH) services, which provides ongoing care to patients from the comfort of your home. 

What we offer

HITH is an option for both private and public patients across the nine towns serviced by West Wimmera Health Service. However, it is voluntary, and you need a referral from your treating doctor and must meet the following criteria.

 

You may be provided HITH if:

  • the doctor treating you in hospital believes the care you need can be safely provided in your home

  • the plan of care is discussed with you and is clearly documented

  • you agree to the plan of care and sign the necessary agreement

  • your home meets the safety requirements for community health professionals to enter and provide the care you need

 

The HITH program provides you with ongoing care in your home, however you will remain under the care of the hospital doctor until they discharge you from the program. Daily care in your home is provided Monday to Friday, with weekends requiring you to visit your nearest hospital for treatment. During the program, you may receive care from nurses, doctors, or allied health professionals.​​

How to access

Only patients admitted to hospital may be referred to the HITH program by their treating doctor.

Cost

There is no cost to patients.
