Post Acute Care
Post Acute Care (PAC) is a State Government funded program that provides short term services to help people recover at home after discharge from a public hospital. The length of the program is usually 28 days (four weeks).

PAC aims to:

Support healing and recovery at home

Fill the gap until longer term services can commence

Provide flexible service that responds to each client's individual needs
What we offer
West Wimmera Health Service offers the following PAC services:
-
Home care services: general house cleaning
-
Personal care: help with hygiene (showering, dressing). To receive this service, Post Acute Care may need to arrange an assessment by the Community Nurse.

Personal Assistance: help with grocery shopping in your local town.

Nursing Care: nurses monitor your care at home, such as wound care, help with medication and other nursing care.

We provide PAC services to people who live in the local areas of:

Natimuk
-
Goroke
-
Nhill
-
Kaniva
-
Jeparit
-
Rainbow
-
Minyip
-
Murtoa
-
Rupanyup
If you live outside these areas a PAC coordinator will refer you to the relevant PAC Program.
How to access
People who are eligible for PAC include those who:
-
have been discharged from a public hospital including day procedure, or the emergency department

are assessed as needing services after leaving hospital
-
have given consent to receive services

Our PAC Coordinator will visit you in the hospital or phone you at home to talk to you about your needs. They will check that you are eligible for the program, and with your consent they will organise the services you require to safely return home.​

Exclusions
Some people are not eligible for PAC. These include patients:
-
without a current Medicare card
-
who are on the Hospital in the Home (HITH) program
-
being discharged from a Residential Aged Care facility
-
admitted to Transition Care Program
-
who have been a patient in a private Hospital
Cost
There is no charge for PAC services, however you may be charged for extras such as wound dressings if required.
